About The Position

Presbyterian Homes & Services – Optage Hospice is looking for an experienced registered nurse who feels called to hospice and wishes to serve older adults within a caring Christian environment. We are seeking a registered nurse to join our Admissions Team as a Hospice Admissions Nurse. In this position you will play a critical role in caring for older adults at the end of life - not only physically, but emotionally, socially, mentally, and spiritually. This is a full-time day position Monday through Friday, and includes participation in an equitable on-call or weekend rotation, along with a holiday rotation. Our clinicians enjoy: a thorough and supportive onboarding process, manageable daily workload assignments that allow provision of high caliber patient and family care with thoughtful consideration to geographic location(s) and patient/dynamic acuity, routine education and CEU offerings, company provided cell phone and technology devices, and mileage reimbursement at the federal rate. Requirements

  • Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Excellent interpersonal skills.
  • Ability to work with dying patients and caregivers in the home, hospital, and other facility settings.
  • Ability to work as a member of an Interdisciplinary Team.
  • Strong oral and written communications skills.
  • Reliable transportation, current driver’s license and current auto insurance.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Nice To Haves

  • Strong desire to serve older adults with a passion for end-of-life care.
  • A minimum of three years of nursing experience preferred.
  • Critical care/oncology, long-term care, or medical/surgical background with strong knowledge of pain and symptom management preferred.
  • Hospice/Home Care experience is a plus, but not required.
  • Strong assessment skills, experience with symptom management preferred
  • Computer skills helpful.

Responsibilities

  • Explaining the Optage Hospice Program, benefits, and services to patients and their families/caregivers.
  • Facilitating and managing all aspects of the hospice admission process including appropriateness for admission.
  • Developing an individualized plan of care, related to the patients' needs, in collaboration with the physician, patient, and their families/caregivers.
  • Determining the appropriate utilization of resources based on clinical assessment, regulations and established clinical best practices.
  • Providing ongoing assessments of physical, psychological, social, and spiritual needs of the patient and family.
  • Providing education, supervision and counseling to the client and their family/caregiver regarding care needs as appropriate to client’s needs.
  • Providing accurate and appropriate documentation of patient/family services.
