About The Position

Admissions Nurse (RN) | Auburn Crest Hospice Be the First Person a Family Trusts The admission visit is the moment a family stops feeling alone. Done well, it changes everything that follows. We are looking for an experienced RN who can walk into a hard room, explain hospice with clarity and compassion, and give families confidence in the care ahead.

Requirements

• Active RN license in Idaho
• Two or more years of nursing experience
• Expertise in pain and symptom management and end-of-life care
• Working knowledge of Medicare, Medicaid, state, and accreditation requirements
• Strong clinical documentation and communication skills
• Valid driver's license, vehicle insurance, and reliable transportation

Nice To Haves

• Hospice or home health experience strongly preferred
• Experience in palliative care, oncology, or long-term care
• Schedule flexibility, local travel, and participation in the on-call rotation

Responsibilities

• Perform hospice consultations and admissions, assessing eligibility and appropriateness of care
• Educate patients and families on hospice services and patient rights, with patience and honesty
• Communicate with physicians to secure admission orders, medications, and recertifications
• Participate in the Interdisciplinary Team Conference and plan of care review every 14 days
• Support documentation review to ensure patients continue to meet hospice qualifying criteria
• Provide clinical supervision, competency evaluations, and orientation support for nursing staff and CNAs
• Contribute to our QAPI program and to in-service education for staff and community partners
• Carry a part-time case load as assigned
