This role is responsible for the admission to hospice services and leads the interdisciplinary care team specific to each patient and family on their caseload. The nurse will obtain history, complete physical assessments, and evaluate appropriateness for hospice services. They will meet with patients and families to explain the hospice program, obtain informed consent, and verify verbal certification of prognosis. The position also involves reviewing insurance/Medicare eligibility and benefits, assessing patient and family readiness for hospice care, and developing the plan of care in collaboration with the interdisciplinary team. Coordination of services, arrangement of necessary equipment and supplies, and effective communication with the IDT are key aspects of this role. The nurse will also facilitate appropriate placement in hospice contracted facilities if necessary, coordinate the delivery of medications, supplies, and home medical equipment, and demonstrate effective communication, feedback, and conflict resolution skills. Additionally, the role includes developing and providing hospice education to inpatient hospital and facility staff and acting as an expert resource regarding hospice and palliative care services.
