The Hospice Admission Registered Nurse provides compassionate, patient-centered admission services to individuals and families in private residences, assisted-living communities, and other home settings. The nurse evaluates hospice eligibility, explains the hospice philosophy and available services, completes the admission process, develops the initial plan of care, and coordinates a safe transition to the interdisciplinary hospice team. The Hospice Admission RN collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to provide holistic care tailored to each patient’s unique situation. This role requires strong clinical expertise, excellent communication skills, and the ability to make independent decisions in dynamic environments. Ultimately, the Hospice Field RN contributes significantly to improving quality of life and facilitating a peaceful end-of-life experience for patients and their loved ones.
