About The Position

FreedomCare is a healthcare company dedicated to revolutionizing the home care industry. We support patients by empowering them to choose caregivers for their own homes. Our mission spans coast to coast, supporting patients across the U.S. We are looking for a HomeCare Registered Nurse (RN) for our Georgia team. This is a remote position in the Georgia area but may require visits to one of our closest FreedomCare Offices in Georgia. The candidate is required to reside in the state of Georgia. Adult Foster Care is a caregiving arrangement where a patient lives with a principal caregiver who provides daily support based on the patient's needs. The goal is to provide necessary care while fostering independence in a home environment, offering a range of care options as needs evolve. This service provides alternatives to nursing facility-level care for individuals who meet those needs, while preserving dignity, self-respect, and privacy. In this position, as a Registered Nurse, you will perform clinical evaluations in the home of the Adult Foster Care member to assess patient needs for enrollment, service plan development, and ongoing compliance. You will also conduct monthly visits with members On Care with FreedomCare to ensure quality service and address any condition changes.

Requirements

Registered Nurse licensed by Georgia Board of Registration in Nursing
Minimum of 2 years clinical experience
1 year of experience with elders or adults with disabilities
Valid driver’s license with an insured vehicle for travel within the state
Strong communication and interpersonal skills.
Problem-solving and decision-making abilities.
Organizational and multitasking skills.
Excellent communication, critical thinking, and organizational skills
Self-motivated
Able to thrive in a fast-paced start-up environment

Nice To Haves

Bilingual in Spanish is a plus
Experience with CRM (Salesforce is a plus)

Responsibilities

Provides support to the caregiver and member, and reviews services according to scope of Adult Foster Care.
Respects client and employee rights and privacy, ensures the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
Follows the FreedomCare’s clinical policy and procedures.
Assesses and regularly reassesses the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.
Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.
Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.
Interprets the philosophy and objectives of the agency to clients, families, and others.
Performs other job-related duties as may be required or directed by the supervisor.
