Registered Nurse (RN) Hospice Homecare
About The Position
The Hospice RN Case Manager – Homecare provides compassionate hospice care to patients in their homes and care facilities. This role is responsible for conducting comprehensive nursing assessments, developing and implementing individualized plans of care, maintaining the highest level of patient comfort and quality of life, coordinating care through an interdisciplinary team approach, performing ongoing reassessments and revising care plans as patient/family needs evolve, and supervising and collaborating with team members to ensure exceptional care delivery.
