Homebound Program Registered Nurse (4299) VALUE BASED CARE BRANN
About The Position
The RN provides expert professional nursing care and primarily delivers care to patients in their own homes as part of a value-based care program. The RN assesses, plans, implements, and evaluates care for patients with urgent, emergent, or potential health problems, with a focus on improving clinical outcomes, care coordination, and patient experience while reducing unnecessary utilization. The RN collaborates with interdisciplinary teams to support chronic disease management, care transitions, preventive care, and population health initiatives, and participates in the management of the department.
Requirements
- Current state licensure required
- BLS certification
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Must be able to make home visits to patients/families in a variety of settings.
Nice To Haves
- Formal training indicated by a graduation certificate from an accredited RN school of nursing
- Experience in clinic-based nursing
- Dedication to patient satisfaction
- Ability to communicate effectively
- Ability to work compatibly with physicians and other staff members.
