Requirements
- Valid Registered Nurse (RN) license in the state of Michigan.
- Minimum of 1 years nursing experience, preferably in home health.
- Kinnser Experinces is a plus
- Strong assessment, clinical, and critical thinking skills.
- Excellent interpersonal and communication abilities.
- Reliable transportation and ability to travel to patient homes in Wayne County, MI.
- Current CPR and Basic Life Support (BLS) certification.
Responsibilities
- Conduct in-home patient assessments and develop individualized care plans.
- Provide skilled nursing services such as wound care, medication management, and chronic disease monitoring.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated patient care.
- Educate patients and their families on managing health conditions and promoting wellness.
- Document all patient interactions, treatments, and progress accurately and timely.
- Maintain compliance with all state and federal regulations governing home healthcare.
