About The Position

Make a meaningful difference in the lives of older people and people living with disability as a Home Support Officer with Country Health Connect. Working within a primary health care framework, you will coordinate, deliver and evaluate services that help clients remain independent, connected and supported within their own homes and communities. This diverse and rewarding role combines client assessment, care planning, service coordination, staff supervision and stakeholder engagement. You will work closely with clients, families, care workers and community partners to ensure services are tailored to individual needs and contribute to positive health and wellbeing outcomes.

Requirements

  • Demonstrate experience working with older people and people living with disability.
  • Have the ability to organise, deliver and evaluate services tailored to individual client needs.
  • Possess strong communication, negotiation and conflict resolution skills.
  • Demonstrate experience supervising staff and working effectively within a multidisciplinary team environment.
  • Have knowledge of aged care, community care and services that support frail aged and young disabled community members.
  • A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
  • NDIS Worker Screening Check required.
  • Immunisation Risk for this position is – Category B
  • SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy
