About The Position

The PRS Level 2 Case Worker supports UC and their sponsors who present moderate risk factors and need in- depth case management. This position involves conducting both virtual and in-person visits, implementing safety plans, and ensuring follow-up on legal, educational and mental health needs.

Requirements

• Bachelor’s degree in social work, Psychology, Human Services, or a related field
• Minimum of 1 year of case management or youth services experience
• Experience working with immigrant children or vulnerable populations
• Strong report writing and crisis intervention skills
• Knowledge of community resources and referral systems
• Bilingual in English and Spanish (spoken and written required)
• Driver’s License
• Authorized to work in the U.S.
• Ability to work in-person in New York City (NY) for at least 3 days or more per week.
• Ability to travel to other Rising Ground sites (if required)
• Frequent local and interstate travel
• Available evenings and weekends as needed

Responsibilities

• Conduct comprehensive in person and virtual visits to assess safety and service engagement
• Submit timely, accurate PRS reports and documentation
• Collaborate with community service providers and stakeholders
• Create and monitor safety and supervision plans
• Participate in step-up meetings when higher-level services are needed
• Address moderate challenges such as school enrollment issues, immigration court compliance, or housing instability
