About The Position

Gunnison Valley Health is seeking a Home Health Nurse to join their small and specialized home care health team. This role involves providing exceptional care to patients in their homes. The organization is a community-owned health system serving Gunnison County and surrounding areas, offering a wide range of services from birth to senior care, and focusing on wellness programs, medical education, and illness prevention. The position offers flexible scheduling with a Monday-Friday day shift, the opportunity to work autonomously within a knowledgeable team, and up to 28 days of paid time off per year.

Requirements

  • Must possess, as a minimum, a Nursing Degree and a current RN license from the state of Colorado.
  • Must have a minimum, one (1) year experience in a hospital, skilled nursing facility, or other comparable institution.
  • Licensed RN in the State of Colorado.
  • Valid Driver’s License, current automobile insurance, reliable transportation, and current CPR certification.

Nice To Haves

  • Wound care experience a plus.

Responsibilities

  • Administer skilled nursing care to patients requiring intermittent professional nursing services in accordance with department policies and procedures.
  • Complete initial evaluation visit, regularly re-evaluate the patients nursing needs, initiates the plan of care and furnishes services.
  • Coordinates services works with physicians and other personnel of changes in patients condition counsels patient and families.
  • Participates in in-service programs, and supervises and teaches other nursing personnel.
