About The Position

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking Registered Nurses (RNs) to join their award-winning team. This role involves designing and implementing patient-specific care plans, providing direct nursing care, monitoring patient conditions, and coordinating health promotion and prevention efforts. The RN will maintain accurate clinical records, including OASIS documentation, and serve as a liaison between patients, physicians, discharge planners, and community health resources. The position also requires effective communication with clinical managers and colleagues, providing training to patients and families, and ensuring patient rights are upheld.

Requirements

  • Current RN license (state-specific).
  • Valid BLS certification.
  • Minimum one year of experience in home health or a healthcare facility.
  • Familiarity with Medicare and Medicaid guidelines.
  • Solid understanding of professional nursing principles and home health documentation.
  • Excellent communication, organizational, and time management skills.
  • Valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.

Responsibilities

  • Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health.
  • Provide high-quality nursing care in alignment with the patient’s care plan, including comprehensive health and psychosocial assessments to support recovery.
  • Monitor patients’ conditions and coordinate health promotion, prevention, and direct care.
  • Maintain detailed and accurate clinical records, including all required OASIS documentation.
  • Communicate effectively with clinical managers and colleagues to ensure high-quality care.
  • Provide training and support to patients and their families/caregivers.
  • Function as a key point of contact between patients and community health resources.
  • Work alongside physicians and discharge planners to provide health education to patients and caregivers.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
  • Understand and adhere to the agency’s process for managing and addressing customer complaints.
  • Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
  • Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
