About The Position

Venture Forthe is seeking a Registered Nurse to provide high-quality home infusion therapy to patients in Rochester and surrounding areas. The role combines hands-on clinical care, patient education, care coordination, and collaboration with providers, pharmacies, and interdisciplinary teams. The nurse travels to patients’ homes and community settings and may need to lift and move equipment and supplies. Requirements

Current New York State Registered Nurse (RN) license.
Graduate of an accredited nursing program.
Strong communication, organizational, and critical thinking skills.
Ability to work independently in varied home environments.

Nice To Haves

At least 1 year of acute care nursing experience is preferred.
Infusion therapy experience is strongly preferred, including IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, and hydration therapy.
Home care experience is preferred but not required.

Responsibilities

Provide home infusion therapy, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
Assess patients and develop individualized infusion plans of care.
Educate patients and caregivers about infusion therapy, medications, line care, and treatment expectations.
Collaborate with providers, pharmacies, and interdisciplinary care teams to support safe and effective care.
Monitor patients for infusion reactions, side effects, and clinical changes.
Maintain accurate and timely clinical documentation.
Support quality improvement initiatives and ongoing competency programs.
Assist with training and supervision of paraprofessional and nursing staff.
