This Home Infusion Pharmacist role is responsible for providing all facets of pharmaceutical care to intravenous homecare patients. This includes assisting with the referral process, providing medical information to patients and caregivers regarding home infusion therapy, performing patient assessment and care, and overseeing the Patient Advocate team. Requirements

  • Bachelor of Science in Pharmacy
  • None years of work experience
  • License to practice Pharmacy in the state of practice
  • Computer skills and dexterity required for data entry and retrieval of information
  • Effective verbal and written communication skills and the ability to present information clearly and professionally
  • Proficient with Windows-style applications, various software packages specific to role and keyboard
  • Strong interpersonal skills
  • Ability to use computer programs necessary for the successful review, authorization, preparation, dispensing and management of home infusion center patients
  • Ability to remain familiar with literatures, references, and resources for therapies and care of patients serviced by the home infusion center
  • Ability to maintain the confidentiality of patients and their personal information at all times
  • Ability to maintain and foster an environment of teamwork and engagement

Nice To Haves

  • Doctor of Pharmacy (PharmD)
  • Completion of a PGY1 or PGY2 Pharmacy Practice Residency
  • 2 years of experience as Pharmacist
  • Experience in specialty pharmacy

Responsibilities

  • Assist with the referral process, including but not limited to the provision of medical information to patients and care givers relative to home infusion therapy.
  • Perform patient assessment and care.
  • Oversee the Patient Advocate team.
  • Compound sterile medications and infusions for the home, outpatient clinics, skilled nursing facilities, and physician/clinic environments.
  • Initiate infusion therapies with successful on-going evaluation of care and completion of required documentation relative to processing therapies, both fiscal and legal.
  • Assist in the teaching of patients prior to discharge to home.
  • Coordinate deliveries with nursing and support services.
  • Act as a pharmacist in charge and make decisions while on duty in the absence of the PIC.
  • Participate in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance, and hazardous waste control.
  • Be responsible for emergency on-call duties on weekends and weeknights as required by center scheduling.
  • Adapt behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Perform other related duties as assigned.
  • Remain knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensure compliance with all such laws, regulations and standards.
