This position supports patients in Evanston and surrounding areas and is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
Requirements
- LPN license in the state you work
- Graduate from an approved school of practical nursing
- Two years of LPN Experience
- Must possess WCC certification.
- Current driver’s license
- Demonstrated experience with tablets, mobile phones and EMR software
- Legally authorized to work in the United States.

Responsibilities
- Provide management and delivery of patient care plans.
- Consistently meet critical deadlines for charting.
- Self-manage time and schedule.
- Spend approximately 20% of your day driving to/from patient locations.
