As a Home Health Speech Language Pathologist, you’ll provide patient-centered care that improves communication, swallowing, and overall quality of life. This role involves evaluating and treating patients using evidence-based speech therapy practices, developing and adjusting individualized care plans based on patient progress, and educating patients and families to support independence and long-term outcomes. You will collaborate with nurses, therapists, and physicians to coordinate care and complete accurate, timely documentation to support clinical and regulatory requirements. Participation in care planning, case conferences, and quality improvement initiatives is also expected, along with identifying changes in patient condition and adjusting treatment accordingly.
