About The Position

As a Home Health Speech Language Pathologist, you’ll provide patient-centered care that improves communication, swallowing, and overall quality of life. You will evaluate and treat patients using evidence-based speech therapy practices, develop and adjust individualized care plans based on patient progress, and educate patients and families to support independence and long-term outcomes. You will collaborate with nurses, therapists, and physicians to coordinate care and complete accurate, timely documentation to support clinical and regulatory requirements. Additionally, you will participate in care planning, case conferences, and quality improvement initiatives, and identify changes in patient condition and adjust treatment accordingly.

Requirements

  • Master’s Degree in Speech-Language Pathology
  • Active Illinois SLP license (or eligible)
  • Completion of Clinical Fellowship Year (CFY)
  • Minimum 1 year of experience (or strong clinical internship experience)
  • Strong communication, documentation, and time management skills
  • Ability to work independently in a home-based setting
  • Valid driver’s license and reliable transportation
  • BLS Certification (AHA) – required within 6 months of hire (if not already obtained)
