About The Position
Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Speech Language Pathologist (SLP) for a Home Health position providing skilled care to patients in their homes in the Northland area including Platt and Clay Counties.
Requirements
- Active Speech Language Pathologist, SLP license in KS and MO required.
- One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
- A valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
Responsibilities
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
- Educate and empower patients and caregivers on therapeutic exercises
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory
