The Home Health Speech Therapist (SLP) will perform accurate and comprehensive speech, language, and swallowing assessments. This role involves obtaining physician orders for certification and recertification of home speech therapy services. The SLP will ensure timely communication of changes in patients' clinical status to healthcare team members and HHA office staff, both verbally and in writing. They will also coordinate care planning and delivery with physicians and other healthcare team members for optimal patient outcomes. The therapist will deliver appropriate speech therapy modalities, assess and manage patient pain, and provide services in a safe environment that minimizes medical errors. This position supports a culture of safety and quality, addresses cultural practices, and participates in the HHA’s information management and Quality Assurance Performance Improvement (QAPI). The role requires compliance with HHA policies, accurate completion of OASIS data sets, and adherence to all applicable laws and regulations. Documentation, verification of completed visits, and participation in interdisciplinary team conferences are also key responsibilities. The SLP will maintain current knowledge of community resources, demonstrate effective time management, and assure continued competency through professional development.
