About The Position
Home Health Care, Inc. is seeking a dedicated RN Visit Nurse for the St. Paul territory. This full-time position offers a unique opportunity for nurses seeking a career where they can think critically, build meaningful patient relationships, and see the results of their care over time. Unlike traditional hospital settings, home health nursing allows you to care for individuals in their own homes, assisting them with recovery after hospitalization, managing chronic illnesses, preventing unnecessary readmissions, and promoting independence. You will combine the autonomy of field work with the support of a well-established home health organization.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
- Strong clinical judgment and assessment skills
- Excellent communication and patient education abilities
- Self-motivated and comfortable working independently
- Reliable transportation and willingness to travel throughout the St. Paul service area
Responsibilities
- Provide skilled RN home health visits throughout the St. Paul territory
- Perform comprehensive nursing assessments
- Deliver wound care, medication management, injections, and disease-specific treatments
- Educate patients and caregivers on medications, diagnoses, and recovery plans
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Monitor patient progress and communicate changes in condition
- Complete accurate documentation using Homecare Homebase
- Help patients remain safely at home while reducing avoidable hospitalizations
