About The Position

Elara Caring is seeking a Registered Nurse Home Health to provide compassionate nursing care to patients in their homes in Sangamon County. The nurse assesses patient needs, carries out care according to plans of care and physician orders, monitors patient conditions, and works with the care team to support safe, high-quality, continuous care. The role also includes protecting privacy under HIPAA and educating patients and families about care and safety. A $5,000 sign-on bonus is listed. The base salary is $75,000 to $80,000 annually; actual pay depends on factors including education, experience, skills, and internal equity.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
