About The Position
American Medical Home Health Services is seeking a dependable and experienced Registered Nurse (RN) to provide home health visits in Northwest San Antonio and surrounding areas. This PRN/per-visit position offers flexible scheduling and competitive per-visit rates. Patient assignments will primarily be in Northwest San Antonio. The role involves electronic documentation through WellSky/Kinnser, with NestMed AI available to assist with clinical documentation.
Requirements
- Current and unrestricted Texas RN license
- Minimum of one year of nursing experience
- Current CPR certification
- Reliable transportation, valid driver’s license, and automobile insurance
- Ability to manage visits independently and complete documentation promptly
Nice To Haves
- Home health and OASIS experience strongly preferred
Responsibilities
- Complete comprehensive patient assessments
- Perform skilled nursing visits according to the plan of care
- Complete Start of Care, Resumption of Care, Recertification, and Discharge assessments, Supervisory, Routine visits as assigned
- Communicate changes in the patient’s condition to physicians and the clinical team
- Provide patient and caregiver education
- Submit accurate and timely clinical documentation
- Coordinate care with therapists, providers, and other members of the interdisciplinary team
