About The Position

We are hiring a Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Carlinville area. This position is eligible for a significant bonus potential, including a first-year success bonus and a sign-on bonus. The bonus opportunities are designed to grow with your tenure at the company.

Requirements

• Active Registered Nurse, RN license in the state you work.
• One year of RN experience and the clinical expertise to deliver quality patient care.
• Current driver’s license.
• Ability to spend ~20% of your day driving to/from patient locations.
• Legally authorized to work in the United States.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently.
• Travel to patient homes within assigned territory.
