Requirements
- Active Missouri RN license in good standing
- Strong clinical judgment and documentation skills
- Ability to manage a flexible schedule including admissions
- Comfort with technology and electronic medical records

Responsibilities
- Provide skilled nursing care in the home.
- Guide each patient’s plan of care from start to finish.
- Assess, plan, implement, and evaluate individualized patient care.
- Develop care plans in collaboration with physicians and the interdisciplinary team.
- Complete admissions and ongoing nursing assessments.
- Educate and support patients and families through changing needs.
- Document care accurately and in a timely manner.
