About The Position
CHI Health at Home is seeking a Registered Nurse for a home health role, offering a patient-centered approach with a 1:1 patient-clinician ratio and ample time for comprehensive care. The position focuses on building meaningful relationships, guiding the full healing process, and providing opportunities for professional growth into clinical leadership, operations, or educational initiatives. CHI Health at Home operates as a ministry of humankindness, extending healthcare into personal settings, dedicated to superior patient outcomes, and committed to innovation. The primary service coverage area will be South Metro Denver areas.
Requirements
- Graduate of an accredited nursing program with an active RN license in the state served.
- Minimum of 1 year of nursing experience.
- Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.

Responsibilities
- Be a clinical leader, applying expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.
- Craft individualized care, developing, implementing, and dynamically adjusting comprehensive care plans that promote patient independence and well-being.
- Collaborate and coordinate seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care.
- Empower and educate, serving as a vital resource for patients and their families, providing crucial teaching and emotional support.
- Mentor and guide, overseeing your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
- Innovate with technology, utilizing leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
