About The Position

Amedisys is hiring a full-time, day-shift registered nurse case manager serving the Fayette County territory. The role provides home health care to patients in their homes, where the nurse assesses patients, coordinates care, and supports healing, health, and independence. Compensation is $85,000–$90,000 annually and may vary by geographic market, job-related knowledge, skills, and experience.

The nurse collaborates with the care team to develop and implement plans of care, makes referrals based on patient needs, supervises LPNs and HHAs, and communicates progress to the clinical manager and care team. The position also includes patient and staff education, timely documentation, assigned on-call responsibilities, and participation in clinical development and continuing education.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on identified patient needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
