About The Position

Become a part of our caring community Make a meaningful impact every day as a CenterWell Home Health Nurse. You will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love. You will report to the Clinical Manager. We are offering a $10,000 Sign-On Bonus As a Home Health Registered Nurse, you will: Provide admission, case management and follow-up skilled nursing visits for home health patients, as directed by the Physician and Clinical Manager Coordinate appropriate care, encompassing necessary healthcare personnel (such as Physical Therapists, Occupational Therapists, Licensed Practical Nurses, Home Health Aides and external providers) Report patient care, condition and progress to patient's physician and the Clinical Manager Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes Prepare visit or shift reports, updates and summaries of patient records, and confer with other health care disciplines in providing optimum patient care This opportunity will require an alternative Baylor/Weekend full-time schedule, to be discussed during interview (i.e. 12 hour Fri/Sat/Sun, Thu/Fri/Sat, etc) Use your skills to make an impact

Requirements

• Registered Nurse, licensed in the state of North Carolina, without restrictions
• A valid driver's license, auto insurance, and reliable transportation
• Current CPR certification

Nice To Haves

• Home Health, Med Surg, ICU, ER, or Acute Care experience

Responsibilities
