About The Position

As a therapist at CenterWell Home Health, you'll help patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life. You will report to the Clinical Manager.

Requirements

• Current and unrestricted Occupational Therapy license in the state of North Carolina
• A valid driver's license, auto insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Confer with physician and clinical team members to obtain patient information and help develop, implement and revise the therapy treatment program related to daily living, work-related skills, community reintegration, adaptive equipment needs, strength, coordination, range-of-motion, etc
• Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs
• Train staff and family members to manage the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings
• Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff
• Provide Occupational Therapy Assistants and Home Health Aide staff with written instructions and care plan that reflects current plan of care as related to therapy
• Oversee Occupational Therapy Assistant and Home Health Aide staff performance, including monitoring the appropriate completion of documentation
