As a Registered Nurse (RN) in Home Health, you’ll play a vital role in helping patients navigate their health journey while enjoying the autonomy, flexibility, and purpose that comes with caring for patients one-on-one.
Requirements
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Minimum of 1 year of clinical nursing experience
- Strong assessment, critical thinking, and communication skills
- Ability to work independently and manage a patient caseload
- Willingness and ability to travel within assigned territory (50%)
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to lift/move up to 50 - 100 lbs and assist with patient mobility
Nice To Haves
- home health experience preferred
- Associate or bachelor’s degree in nursing (BSN preferred)
- Experience with HomeCare HomeBase (preferred)
Responsibilities
- Deliver skilled nursing care to patients in their homes in accordance with physician orders and the individualized plan of care
- Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
- Develop, implement, and update personalized care plans based on patient needs and progress
- Provide education and support to patients, families, and caregivers to promote independence and improve outcomes
- Monitor patient conditions and communicate changes to physicians, Clinical Supervisors, and care team members
- Coordinate care with an interdisciplinary team to ensure continuity and quality of care
- Accurately document patient visits, assessments, and care provided in accordance with company standards and regulatory requirements
- Ensure compliance with HIPAA and maintain strict patient confidentiality
- Promote a safe environment by identifying and addressing potential risks in the home setting
