As a Registered Nurse (RN) in Home Health, you’ll play a vital role in helping patients navigate their health journey while enjoying the autonomy, flexibility, and purpose that comes with caring for patients one-on-one.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in the state of practice
  • Minimum of 1 year of clinical nursing experience
  • Strong assessment, critical thinking, and communication skills
  • Ability to work independently and manage a patient caseload
  • Willingness and ability to travel within assigned territory (50%)
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to lift/move up to 50 - 100 lbs and assist with patient mobility

Nice To Haves

  • home health experience preferred
  • Associate or bachelor’s degree in nursing (BSN preferred)
  • Experience with HomeCare HomeBase (preferred)

Responsibilities

  • Deliver skilled nursing care to patients in their homes in accordance with physician orders and the individualized plan of care
  • Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
  • Develop, implement, and update personalized care plans based on patient needs and progress
  • Provide education and support to patients, families, and caregivers to promote independence and improve outcomes
  • Monitor patient conditions and communicate changes to physicians, Clinical Supervisors, and care team members
  • Coordinate care with an interdisciplinary team to ensure continuity and quality of care
  • Accurately document patient visits, assessments, and care provided in accordance with company standards and regulatory requirements
  • Ensure compliance with HIPAA and maintain strict patient confidentiality
  • Promote a safe environment by identifying and addressing potential risks in the home setting
