About The Position

Hillcrest Health & Living is seeking an experienced, compassionate, and dependable Home Health Registered Nurse to join their Hillcrest Home Health team. This regular casual position offers flexibility, ideal for an RN who enjoys working independently, building one-on-one relationships with patients, and providing skilled nursing care in the patient's home. The schedule includes a mix of planned and PRN daytime visits, primarily as part of a weekend rotation, with opportunities for additional weekday or after-hours on-call work. Requirements

  • At least one year of nursing experience.
  • Graduate of an accredited nursing program required.
  • Active Registered Nurse license in Nebraska or another Nurse Licensure Compact state required.
  • Current Basic Life Support certification required.
  • Valid driver’s license required.
  • Current automobile insurance coverage required.
  • Reliable transportation required.
  • Must be able to speak, read, and write English.
  • Must be able to safely provide nursing care in a variety of home environments.
  • Must be able to perform repetitive sitting, bending, standing, reaching, lifting, and movement as needed.
  • Must be flexible, calm, and professional in stressful or changing situations.
  • Must be able to work independently and make sound clinical decisions in the field.
  • Must be able to manage a mix of scheduled and PRN visits.
  • Must be able to utilize technology, electronic documentation systems, and clinical resources efficiently and effectively.
  • Must be able to travel throughout the Omaha metro area.

Nice To Haves

  • Previous home health or home care nursing experience preferred.
  • Experience with catheter management, wound care, IV therapy, and disease-process education.
  • Previous OASIS experience preferred.
  • Strong clinical assessment and critical-thinking skills.
  • Ability to solve practical problems and respond appropriately to changing patient needs.
  • Strong communication and relationship-building skills with patients, families, providers, and team members.
  • Ability to work independently while remaining connected with the interdisciplinary care team.
  • Strong organizational, time-management, and documentation skills.
  • Comfort using computers, laptops, electronic medical records, and clinical documentation systems.
  • Reliable follow-through and a strong commitment to patient-centered care.
  • One year of nursing experience in a home care or home health setting preferred.

Responsibilities

  • Complete skilled nursing visits throughout the Omaha metro area.
  • Assess patients’ health status, clinical needs, home environment, and progress toward established goals.
  • Provide catheter management, wound care, IV therapy, medication support, and disease-process education.
  • Educate patients and family members on care plans, medications, symptoms, safety, and strategies that support independence at home.
  • Develop trusted relationships with patients and families through compassionate, respectful, and dependable care.
  • Communicate changes in condition, concerns, and recommendations to providers and interdisciplinary team members.
  • Collaborate with physicians, therapists, social workers, caregivers, and other clinical partners to coordinate care.
  • Complete accurate and timely clinical documentation in accordance with agency standards and regulatory requirements.
  • Support scheduled and PRN daytime visits as part of the weekend rotation.
  • Pick up additional weekday or after-hours on-call visits based on interest and availability.
  • Use sound clinical judgment and problem-solving skills while working independently in the field.
  • Promote Hillcrest Home Health and its mission through professional interactions with patients, families, providers, and community partners.
