About The Position

This role involves case management, conducting starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services. It also includes occasional new hire orientations, in-service training, supervisory reviews, and contributing to training, education, care practices, outcomes, and overall business success. The position requires occasional backup for Skilled Nursing visits in most areas. The role is a field staff leadership position with opportunities for career advancement within the agency.

Requirements

  • At least 1 year of experience in Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments.
  • At least 1 year of paid experience at a home health agency (or equivalent).
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date).
  • 1 year Experience in case management required.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience.

Nice To Haves

  • Nurse Supervision experience is a plus.
  • 2-5 years' experience is preferred.
  • Home care experience preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong attendance and dependability track record with previous employers.

Responsibilities

  • Conduct case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services.
  • Provide occasional new hire orientations, in-service training, and supervisory reviews.
  • Contribute to training, education, care practices, outcomes, and overall business success.
  • Perform occasional Skilled Nursing visits as backup.
  • Coordinate and provide patient care, ensuring optimal outcomes.
  • Oversee patient services and provide expert nursing care.
  • Ensure successful communication with physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to the care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
