About The Position

Home Care Network, Inc. (HCN) is a nurse-owned and locally led home health agency seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) to join our agency and serve existing and returning clients throughout Stark County and nearby counties across Northeast Ohio in a part-time or full-time capacity. This is a field staff leadership role where the RNCM has a passion for delivering consistently excellent care and care experiences to clients, and the ability to do so directly and through the thoughtful leadership of others. This position requires travel to multiple counties but offers a consistent schedule to fit personal/family commitments (no overnight shifts or overnight travel). For those pursuing career growth, a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported. As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes. With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client and his/her family.

Requirements

  • At least 1 year of experience in Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments.
  • At least 1 year of paid experience at a home health agency (or equivalent) is required.
  • Minimum of one (1) years of nursing supervisory experience.
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management required.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willing to consent to a background check, including a criminal record check, and an employment and education verification.

Nice To Haves

  • Nurse Supervision experience is a plus.
  • 2-5 years' experience is preferred.
  • Home care experience preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.

Responsibilities

  • Case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services.
  • Occasional new hire orientations, in-service and supervisory reviews.
  • Input to influence training and education, care practices and outcomes, and the success of clients, employees and business overall.
  • Back-up for occasional Skilled Nursing visits in most areas.
  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
