Requirements
- Active Arizona RN or LPN license (or Compact License)
- Valid Arizona Driver’s License
- Current BLS Certification (online-only courses not accepted)
- Current TB test or chest X-ray (within 12 months)
- Level One Fingerprint Clearance Card
- Article 9 Training (assistance available)
- Strong communication, time management, and problem-solving skills
Responsibilities
- Provide skilled in-home nursing care for low- to high-acuity patients
- Perform comprehensive patient assessments and follow individualized care plans
- Administer medications and treatments as prescribed
- Collaborate with families and interdisciplinary team members
- Accurately document care and report changes to the RN Clinical Supervisor/Site Director
- Safely assist with lifting, positioning, and transferring patients
