About The Position
Perform patient assessments and develop individualized care plans. Deliver skilled nursing care, including medication and treatment management. Monitor patient progress and adjust care as needed. Educate patients and caregivers on health management and self-care. Coordinate with physicians and interdisciplinary teams. Supervise Home Health Aides and LPNs. Requirements
- Active RN license
- BSN + 1 year experience OR ASN/Diploma + 2 years experience
- Current CPR certification
- Valid driver’s license and reliable transportation
