Home Health RN Case Manager

Requirements
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Minimum of one (1) year of nursing experience in a clinical care setting
- Strong assessment, critical thinking, and communication skills
- Ability to work independently and effectively manage a patient caseload
- Reliable transportation, a valid driver's license, and current auto insurance
- Willingness to travel throughout the assigned service area
Nice To Haves

- Hospice experience
- Experience with HomeCare HomeBase (HCHB)
- Associate's or Bachelor's Degree in Nursing
Responsibilities
- Provide skilled nursing care to patients in their homes
- Perform comprehensive patient assessments and develop individualized plans of care
- Monitor patient progress and adjust care plans as needed
- Educate patients, families, and caregivers on disease management, medications, treatments, and safety practices
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate care
- Complete timely and accurate clinical documentation
- Identify changes in patient conditions and communicate findings to the care team
- Participate in case conferences and interdisciplinary care planning
- Ensure compliance with all regulatory, agency, and professional nursing standards
- Maintain patient confidentiality and HIPAA compliance at all times
