Requirements

  • Current Arizona Registered Nurse (RN) License (or Compact License eligible for Arizona)
  • Graduate of an accredited nursing program
  • Current CPR Certification
  • Valid Arizona driver's license
  • Reliable transportation and current auto insurance
  • Currently licensed as a Registered Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.

  • Case management experience
  • Experienced RNs who are interested in transitioning into home health are encouraged to apply.

Responsibilities

  • Manage an assigned caseload within the dedicated Scottsdale territory
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
  • Perform comprehensive nursing assessments and develop individualized plans of care
  • Provide skilled nursing care, medication management, wound care, and patient education
  • Monitor patient progress and communicate changes with physicians and the care team
  • Coordinate care with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides
  • Complete timely and accurate documentation using our electronic medical record system
  • Help reduce hospitalizations while improving patient independence and quality of life
