Home Health RN Case Manager - Scottsdale | $85,000 to $100,000 + Sign-On Bonus

Requirements
- Current Arizona Registered Nurse (RN) License (or Compact License eligible for Arizona)
- Graduate of an accredited nursing program
- Current CPR Certification
- Valid Arizona driver's license
- Reliable transportation and current auto insurance
- Currently licensed as a Registered Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.

Responsibilities
- Manage an assigned caseload within the dedicated Scottsdale territory
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Perform comprehensive nursing assessments and develop individualized plans of care
- Provide skilled nursing care, medication management, wound care, and patient education
- Monitor patient progress and communicate changes with physicians and the care team
- Coordinate care with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides
- Complete timely and accurate documentation using our electronic medical record system
- Help reduce hospitalizations while improving patient independence and quality of life
