About The Position

This is a full-time Home Health RN Case Manager position covering Grayslake and surrounding areas. The role involves developing and maintaining therapeutic relationships, providing individualized care, and mobilizing appropriate resources to ensure quality, cost-effective care. The position requires assessing patient and family needs, educating them on care plans, and collaborating with healthcare team members. The RN Case Manager will utilize the nursing process for problem-solving, make sound clinical decisions, and initiate referrals and discharge planning. Leadership, professional development, and continuous learning are also key aspects of this role. The position requires travel within the Advocate service area in all weather conditions and involves visits to private residences.

Requirements

Graduate from an accredited school of nursing
Earn 10 continuing education credits annually
One year of medical surgical experience
Has demonstrated proficiency for the requirements for NC I
Good communication and interpersonal skills
Good documentation skills.
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment
Current Illinois RN license
Current CPR

Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
Exposed to widely varying social and environmental conditions in patient’s neighborhoods and homes.
Must be willing to work occasional overtime if needed.
Required to make visits to private residences which may not be accessible to people with disabilities.
Must be able to assist with ambulation including turning and transferring patients up to 200 pounds
Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
Unique patient lifting/movement situations will be assessed on a case-by-case basis.
