The Home Health RN Case Manager will develop and maintain a therapeutic relationship throughout the healthcare continuum. This role involves providing individualized care to promote physical and psychosocial comfort, assessing patient and family readiness, and involving them in planning and implementing care. The position also focuses on assuring and improving quality care by facilitating the patient and family’s right to receive quality, cost-effective care, mobilizing appropriate resources, adapting practice to the latest standards based on evidence-based literature, and participating in quality/process improvement initiatives. Additionally, the role involves patient/staff education, demonstrating a commitment to meeting learning needs and assisting in creating an environment that promotes educational growth opportunities for nursing peers and colleagues. The Nursing Process is utilized as a problem-solving model, recognizing changes in clinical situations, evaluating and implementing changes to the plan of care, and making sound clinical decisions for patients with complex problems. The RN Case Manager will prioritize and organize patient care, initiate referrals and discharge planning, apply ethical decision-making, monitor for confidentiality, and delegate correctly to unlicensed assistive personnel. Leadership, collaboration, and professional development are key, demonstrating respect and understanding for peers and other clinical disciplines, participating as an effective member of the patient care team, and demonstrating leadership behaviors committed to the development of self and others. This role requires participation in unit meetings and activities that improve patient care or administrative systems, utilizing available resources to maintain a current knowledge base, demonstrating effective communication, feedback, and conflict resolution skills, and critiquing own nursing practice.
