About The Position

Anew Home Health is seeking a dedicated Home Health RN Case Manager to join their growing team in Hendricks County (Brownsburg, Avon, Danville). This role is responsible for delivering patient care through the nursing process of assessment, planning, implementation, and evaluation. The Registered Nurse will plan, organize, and direct home care services, utilizing community resources to meet the needs of individuals and families in their homes. The position emphasizes person-centered care, continuous learning, and making a difference in the lives of others.

Requirements

Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Prolonged or considerable walking or standing.
Able to lift, position or transfer patients.
Able to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling or crouching.
Visual acuity and hearing to perform required nursing skills.

Nice To Haves

One (1) year of home health care experience preferred.
One-year supervisory/management experience with direct care staff preferred.

Responsibilities

Directly supervises Home Health Aides and LPN’s, carrying out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include scheduling, planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems. Performs supervisory visits with Home Health clients and conducts and authorizes inservices.
Conducts client assessments, suggesting level of needed service packages to client and their family.
Develops plans of care with the participation and consent of the client and/or caregiver/family member or legal representative.
Sets up medications for clients and monitors side effects, drug interactions, dosages, and implements changes per physician instructions. Informs client and family of changes immediately. Orders medication for clients from pharmacy.
Listens and documents significant changes in condition of clients, including incidents, unexplained injuries, medication errors, loss of client property, or evidence of client or family dissatisfaction and communicates with supervisor.
Analyzes data gathered from assessment and records reviews, and acts promptly to resolve problems.
Participates in quality assessment and assurance activities including implementation of recommendations and monitoring of results.
Develops plans of care with measurable objectives and timetables and accommodates clients’ needs and preferences including choices about preferred intensity of medical treatment.
Maintains ongoing communication with other disciplines/departments to evaluate client responses to interventions, suggest alternative approaches when indicated and to revise and update plans of care to reflect progress toward goals as directed by supervisor.
Interacts with clients, caregivers/family members, legal representatives, and resolve concerns.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
Instructs, supervises and evaluates home health aide care provided every two (2) weeks or per regulatory guidelines.
Demonstrates C.A.R.E. values to our clients, family members, customers and staff.
Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided, used, and maintained.
