About The Position

We are looking for nurses who want to improve the health and wellbeing of their patients. We have a great opportunity for a Registered Nurse Case Manager in Indianapolis, IN. You will be responsible for assessing, planning, coordinating, and overseeing patient care. This position demands strong clinical expertise, exceptional communication skills, and a commitment to promoting patient well-being and caregiver competence.

Requirements

  • Current RN licensure in state of Indiana
  • Current CPR certification required
  • Current Drivers License, vehicle insurance, and access to a dependable vehicle
  • Strong assessment, care planning, and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative healthcare team.
  • Strong computer skills for documentation and communication purposes.

Responsibilities

  • Conduct comprehensive initial and ongoing assessments of patients' health status, including physical, emotional, and psychosocial needs.
  • Develop individualized care plans in collaboration with patients, their families, and the interdisciplinary healthcare team, addressing specific needs and goals.
  • Coordinate and facilitate the provision of healthcare services, therapies, and medical equipment as prescribed in the care plan.
  • Provide patients and their families with education on their medical condition, treatment options, and self-care techniques to enhance their independence and well-being.
  • Administer medications and monitor medication adherence, side effects, and interactions, ensuring patient safety.
  • Maintain accurate and up-to-date patient records, documenting all assessments, interventions, and changes in the patient's condition in compliance with regulatory standards.
  • Regularly evaluate the patient's progress toward their goals and make necessary adjustments to the care plan as needed.
  • Serve as an advocate for patients and their families, ensuring their needs and preferences are effectively communicated and met within the healthcare system.
  • Collaborate with other healthcare professionals for the benefit of your patient.
  • Participate in quality improvement initiatives to enhance the delivery of care and maintain compliance with regulatory standards.
  • Be prepared to handle medical emergencies, assess the situation, and take appropriate actions to stabilize the patient.
