About The Position

This is a full-time home health nurse position in the north suburban territory covering Evanston, Lincolnwood, Skokie, Morton Grove, and Niles areas. The role involves developing and maintaining therapeutic relationships, providing individualized care, assessing patient and family needs, and collaborating with the healthcare team to ensure quality, cost-effective care. The position requires strong communication, documentation, computer, time management, and organizational skills. The nurse will be responsible for initiating referrals, discharge planning, and coordinating care using an interdisciplinary approach. The role also emphasizes leadership, collaboration, and professional development, including continuous learning and participation in quality improvement initiatives.

Requirements

Graduate from an accredited school of nursing
Earn 10 continuing education credits annually
One year of medical surgical experience
Has as demonstrated proficiency for the requirements for RN role
Good communication and interpersonal skills
Good documentation skills.
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment
Current Illinois RN license
Current CPR
Ability to work weekends, holidays, and different shifts in order to accommodate staffing
Ability to take “on call” assignments.
Ability to handle multiple tasks and complete work within short timeframes.
Ability to transport objects, patients, and equipment
Ability to respond quickly to patient needs and work at fast pace
Ability to respond to patients in a timely manner and access all areas of the unit
Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
Must be willing to work occasional overtime if needed.
Must be able to assist with ambulation including turning and transferring patients up to 200 pounds

A valid driver’s license issued by the Division of Motor Vehicles.
