This position will be hired as PRN or Part-Time only. Additional Part-Time hours are also available with a 2-hour travel radius to "Secondary" locations listed below. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout Southwest Ohio in a PRN or Part-Time capacity. As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes. With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
