About The Position

Ohana Pacific Health is seeking a passionate Home Health Registered Nurse to provide skilled nursing care to patients in their homes. The nurse provides care to patients requiring substantial and specialized nursing skills and works closely with patients, families, physicians, and interdisciplinary team members using a case management model. The role promotes effective, collaborative communication among the disciplines involved in each patient’s care. The Home Health Registered Nurse provides leadership for an interdisciplinary team and direction toward achieving individual patient goals. The role also includes completing a comprehensive history and physical assessment when a patient is admitted to the agency, and consulting with and educating patients and families about health and wellness in the home.

Requirements

Bachelor of Science in Nursing (BSN); an Associate Degree in Nursing (ADN) may be considered instead if the candidate has at least six years of direct patient care experience in home health or a related clinical setting.
Current Registered Nurse license in the State of Hawaii.
BLS Certification.
At least one year of experience in a hospital.

Nice To Haves

Medicare Home Health Agency experience preferred.
