About The Position

Concierge Home Care is seeking Home Health Registered Nurses (RNs) for a role that values time, expertise, and independence, allowing nurses to control their schedule and income. This position offers a flexible schedule and autonomy, focusing on 1:1 patient care in the home within the Port Orange, FL / Volusia County area. Territories are designed to minimize drive time through clustered scheduling. The company emphasizes flexibility, autonomy, efficient technology (AI-assisted charting, user-friendly EMR), and robust growth and clinical support, including structured onboarding, mentorship, preceptorship, ongoing training, and free CEUs. The role involves comprehensive patient care from assessment and care plan development to skilled nursing, patient education, coordination with interdisciplinary teams, monitoring progress, and streamlined documentation.

Requirements

  • Active, unencumbered Florida Registered Nurse (RN) license
  • Valid driver’s license, reliable transportation, and auto insurance
  • Home health experience required

Responsibilities

  • Perform patient assessments and develop individualized care plans
  • Provide skilled nursing care and patient education
  • Coordinate care with physicians and interdisciplinary teams
  • Monitor patient progress and adjust care plans as needed
  • Complete documentation using streamlined, AI-assisted tools
