About The Position
Interim HealthCare is looking for a Home Health Registered Nurse (RN) to join their team and cover the Rochester and surrounding area territory. This role offers the opportunity to reignite your passion for nursing by providing one-on-one care and directing your own day. As a Home Health Registered Nurse, you will provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehab, or nursing facility. You will work with a team of physicians, oversee LPNs, CNAs, and aides, examine patients, perform assessments, document progress, and report changes to their physician. Additionally, you will educate patients and coach family members on the plan of care, medication, therapy, diet, exercise, and home safety, ensuring goals are met and coordinating discharge from services.
Requirements
- Graduate of an accredited nursing program and active RN license in Minnesota
- Minimum of 1 year of nursing experience, ideally in hospice or home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty, strong communication skills and compassion
Responsibilities
