About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. You work with physicians and other clinicians to manage care plans that support recovery. The role involves providing skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans, monitor patients and report changes, educate patients and families, maintain accurate records, coordinate care, and oversee Home Health Aides and their documentation. The position is per diem, with scheduled weekly hours listed as 1. Compensation is listed as a per-visit/unit rate of $49.00–$69.00. The posting also provides a full-time annual pay range of $77,200–$106,200 as a good-faith estimate for 40-hour-per-week employment. The role is patient-facing and requires TB screening. It also participates in Humana’s driver safety program, requiring a valid state driver’s license and personal vehicle liability insurance meeting applicable minimum limits.
Requirements
- Med-surg, ICU, ER, or acute-care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.
- Must complete TB screening if selected.
- Must maintain personal vehicle liability insurance in accordance with state minimum limits or the stated alternative coverage limits, whichever is higher.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
