About The Position
Concierge Home Care is hiring a PRN Home Health Registered Nurse (RN) in the Jacksonville area. This modified full-time role is designed for nurses who want more flexibility, fewer workdays, and higher earning potential—without sacrificing patient care quality. The coverage area includes Jacksonville/Duval /Nassau Counties, with territories designed to minimize drive time with clustered scheduling.
Requirements
- Active, unencunbered Florida Registered Nurse (RN) license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Perform comprehensive patient assessments in the home setting
- Develop and manage individualized care plans
- Provide skilled nursing care and patient/caregiver education
- Administer treatments and monitor patient condition
- Coordinate care with physicians and interdisciplinary teams
- Evaluate patient progress and adjust care as needed
- Complete accurate, timely documentation using streamlined tools
