About The Position
Concierge Home Care is hiring Home Health Registered Nurses (RN). This role offers an opportunity to take control of your schedule and income through a pay-per-visit model with flexible scheduling and autonomy. The position involves 1:1 patient care in the home, with a coverage area including Fernandina Beach, Yulee, FL, and Nassau County, with territories designed to minimize drive time. The company utilizes AI-assisted charting and a user-friendly EMR to reduce documentation time, allowing nurses to focus more on patient care.
Requirements
- Active, unencumbered Florida Registered Nurse (RN) license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Perform patient assessments and develop individualized care plans
- Provide skilled nursing care and patient education
- Coordinate care with physicians and indisciplinary teams
- Monitor patient progress and adjust care plans as needed
- Complete documentation using streamlined, AI-assisted tools
