About The Position

Concierge Home Care is hiring Home Health Registered Nurses (RN). This role offers the opportunity to take control of your schedule and income, with a focus on 1:1 patient care in the home. The coverage area is Jacksonville, FL / Duval County, with territories designed to minimize drive time through clustered scheduling. The position offers a pay-per-visit model with potential for high annual earnings and quarterly bonuses, along with mileage reimbursement.

Requirements

• Active, unencumbered Florida Registered Nurse (RN) license
• Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

• Home health experience preferred, but not required

Responsibilities

• Perform patient assessments and develop individualized care plans
• Provide skilled nursing care and patient education
• Coordinate care with physicians and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Complete documentation using stream lined, AI-assisted tools
