About The Position

We’re seeking a compassionate Registered Nurse (RN) for a full-time Home Health position serving patients throughout Thomaston, Torrington, Harwinton, and the surrounding communities. This is a case management role for home health patients where you will have direct patient scheduling, allowing you to control your start and end times. You will serve adult and geriatric patients in their homes or senior living communities. This is a longstanding, stable company with years of successful operation.

Requirements

• RN license in the state of CT
• Some experience in Home Health required
• US Citizen or Green Card Holder
• Comfortable with traveling between patient homes (expect 20–40 minutes of driving at times)
• Full proficiency in completing OASIS documentation

Nice To Haves

• Experience providing home health care to sub-acute medical/surgical patients with Medicare payer source

Responsibilities

• Case management role for home health patients

• Willing to cover hospice overflow
